Picture Archiving and Communication Systems (PACS), introduced in the 1980s and achieving widespread adoption by the 2000s, form the technological backbone of modern diagnostic imaging. These systems manage the acquisition, storage, retrieval, and display of medical images, processing billions of studies annually across global healthcare networks. The integration of AI into these established systems requires careful consideration of technical standards, workflow implications, regulatory requirements, and organizational change management.

The remaining challenge is systems engineering: designing and implementing integration architectures that deliver AI capabilities to radiologists in ways that enhance rather than disrupt clinical workflows.
